We like to ensure the referral process is efficient, rapid and reliable. In order to make the service quick and efficient please follow this link, which allows you to complete an online referral form to be emailed directly to us.
Alternatively you can print out the downloadable PDF referral form and fax or post it to us. We ask you to kindly forward up to date contact details for your patients with e-mail addresses for you and them, where possible to allow for better communication.
We accept referrals for:
- Dental Implants
- Endodontics
- Periodontics
We are happy to offer an OPG referral service for your patients of full or sectional OPG’s.
Referred patients can normally be seen within 48 hours of referral.
Radiographs can be supplied via e-mail or printed and posted if preferred within 24 hours of the appointment.
The fee for this service is £30.
